7. Relevant Domicile Certificate (for candidate who has done MBBS outside West Bengal)
8. MBBS Degree Certificate
9. 3RD Prof Part II MBBS Mark-sheet
10. Permanent / Provisional Registration Certificate (as applicable)
11. Appropriate Caste certificate / EWS Certificate (as applicable). OBC certificate as well as EWS certificate should be updated one issued during 2020 or 2021.
12. PWD Certificate from IPGMER, Kolkata (As applicable)
13. Internship Completion Certificate (as applicable)
14. Proof of previous Degree / Diploma Certificate with completion date (As applicable)
15. Indemnity Bond & Discontinuation Bond prepared locally on non judicial stamp paper of not less than Rs. 10 (For format of bond click here)
